By signing this contract all parties agree to abide by all the guidelines set by both the state of Md. and Laurie Heisler for ACW Childcare and substitutes. All guidelines are located in the parent handbook for you to read and review. Please be sure to read all of the handbook as it has very important information listed in it as well as Paid closed dates, procedures, parent fees and substitute information.

Childcare will be provided on the days checked below and no other days will be granted to anyone unless space permits and has been discussed with Laurie Heisler.

Hours of your contract are from _________to_________ and anytime prior to drop off or after pick up contract hours will be charged  $15.00 for the first 10 minutes and $1.00 for each additional minute that your child is in my care unless pre arranged with provider. ACW will not tolerate continued lateness, as it is your responsibility to pick your children up on time.

Your childcare tuition is $______________per week, and is due on Friday prior to the upcoming week and when childcare is closed on Friday your Tuition will be due the last day childcare is open, this includes Holidays, vacation and sick days as well.

Your transportation fee that is due weekly, this is a set rate and is due per week and will not be adjusted if your child is absent for any reason including school closing dates.

B&A School  7:00am-8:30 and 3:30-5:00pm rate:

$125.00 per week or $_____________ No charge if school closes early, opens late or is closed for a holiday/teacher day for full time children, part time children will be charged $35.00 extra per day school is closed.

·        Before School- Will only be provided between 7:00am-8:30 when space permits $65.00 per week-Part Time care $35.00 extra per day when AACP school is closed if space permits.

·        After School-Will only be provided between 3:30pm-5:00pm when space permits $65.00 per week-Part Time care $35.00 extra per day when AACP school is closed is closed if space permits.

Before & After School Children  

• No summer care will be provided for non napping children.
• Transportation will not be provided in the event of snow or ice on the roads.
• See contract for details on rate and care when school is closed.
• Summer care will only be provided to part time children if space permits.
• No rates will be adjusted for part time care for any reason even if your child is absent or may not be able to come due to closings of schools. 
• When AACPS are closed your childcare will still need to be paid even if your child does not attend school age childcare.
• Part time childcare is base on the available open spaces that are provided by the contracts of other children in care and you are not guaranteed any other times other then your stated contract times.
• If school opens late, closes early or is closed and your child is not registered full time then ACW may not have space open for your child to come all day or part of the day and you are responsible to find care for that day.
• When school is dismissed early or opens late you will need to pick your child up from school or take them to school unless there is a available space that day for your child to come earlier then contracted time.
• All children will be permitted to walk to and from the bus stop daily with out provider.
• Children 6 years and older will be allowed to play outside with out provider, but will be checked on every 15 minutes as stated by law.
• See game section of handbook for rules on video games.
• No care when AACPS are closed unless discussed with provider and arrangements have been made between the two of you.
• If your child becomes destructive your childcare could be terminated or you could be charged with damages.
• No profanity-this language will be tolerated for any reason.
• If your child is repeatedly disrespectful termination could be in order and you will need to find another daycare.

Homework 

• All children will be given 30 minutes to do homework each day if your child states they don’t have homework then ACW will not go thru books to see if they do.
• All children are responsible for their own homework completion.
• Please check your child's homework nightly, as ACW will not have the time to check over homework. 
• Parents must sign any and all forms sent home from school as ACW will not be able to sign anything involving your child's schoolwork.

Transportation

Our program includes neighborhood walks field trips, play dates and other out of house activities that involve transporting of children by foot, in strollers, wagons, or even in my vehicle. I will take all possible precautions when transporting the children. All children must wear seat belts and or be in a booster / car seat as REQUIRED BY LAW. If I need a child seat I will request to use yours for the time being. Any child under 4 years of age will need to wear a safety 1^st. waist/wrist belt to ensure there safety during any kind of walks or hold onto a walking rope - Each belt will be connected to a friends belt so they can start practicing there walking skills at an early age, Children need to learn the safety of crossing the road or walking in the Street before they will no longer need a waist/wrist belt, once they have mastered this skill they will be allowed to holding the walking rope.

Surrounding Area Schools

Transportation Charge

Schools other then High Point Elementary & George Fox Middle will have a transportation fee. This charge can range from $7.00-$40.00 per week depending upon the schools distance from ACW and the cost of gas. This fee will not exceed $40.00 and is due weekly.

Bus Transportation

AACPS provides bus transportation to and from High Point Elementary by picking up and dropping off at the corner of 204th and Outing Ave. ACW will provide transportation for High Point, but in the event transportation is not available then ACW will have the children take the bus to and from school. All parents with school age children agree to their children walking to and from the bus stop unassisted by an adult.

               10. This contract will be reviewed and updated every march or when the provider feels that changes need to be made.

               11. Backup childcare will be the parent’s responsibility not ACW/Laurie Heisler in the event that your child becomes sick or ACW would need to close.

                12. I understand and I will allow ACW/Laurie Heisler to transport my child in her childcare van from time to time or as needed.

                13. I understand and I will allow ACW/Laurie Heisler to let my child 6 years or older play outside time to time unattended and I understand they will be checked on as required by law every 15 minutes.

                15. I hear by give my consent for ACW/Laurie Heisler to take pictures of my child and place them on ACW web site, give to other parents when their child is in them and use them to advertise my business.

                 16. ACW will accept a check for payment from parents only when the parents agree to pay all fees related to Laurie/James Heisler’s BB&T checking account. These fees will include a $25.00 bounce fee and any fees that are charged to their BB&T checking account by other companies do to insignificant funds from your check not clearing. If your check bounces then ACW/Laurie Heisler will not take payment in check from parents any longer.

                  17. I understand that all substitutes will be referred to as ACW in this contract and in the handbook and they will provide care for my child from time to time or when needed.

                  18. By signing this I’m/we are in sound mind and body and admitting that no one forced me/us to sign this contract and have read and understood all parts of this contract/handbook and agree to abide by the contract/handbook at all times.
